Church OutreachThe Charlotte REACH 2010 Project consulted with Presbyterian Hospital's Parish Nurse Program to provide consultation and technical assistance in the development of Church Outreach programming in Faith Based congregations in the Northwest Corridor of Charlotte. Congregations often provide programs and activities within congregations that rely on and are dependent upon services and service providers outside of the congregations. The Church Outreach component of the Charlotte REACH 2010 Project is unique in that it encourages congregations to tap the resources within the congregation to provide ongoing programming and activities for the congregation. REACH 2010 staff and the parish nurse educate individuals and groups referred from congregations and LHAs to help people to learn to take responsibility for their health and wellbeing. Although they do not provide in-home health care services, they support the efforts of LHAs who provide door-to-door outreach as educators and change agents in the communities. Finally, they are liaisons to the Charlotte REACH 2010 programs, public health, and community service agencies to enhance healthcare delivery to members of the congregation and the Charlotte REACH 2010 targeted communities. The effort of health and wellness promotion in the faith-based community is a project within itself. There are countless opportunities to impact change in health behavior in the faith-based and surrounding communities. The Charlotte REACH 2010 Project continues the efforts of congregations that historically utilized the church as a place for health and wellness, healing, knowledge and education second only to a formal education. Designing such an approach in the faith-based community must be simple and strategic. Focuses should be limited and should employ the interests of the congregation first to ensure goals and objectives of projects are met. As churches continue to grow and become major businesses in the community, the chance to position this concept and model as a force in the elimination of health disparities is likely and highly recommended.
